Known to exist — copy sought

These items were first listed as wanted documents for the public catalogue (jordanforge-infrastructure issue 10). A September 2026 public-source pass looked for independent bibliographic evidence. Where a catalogue record, a citation from the time, or a later official page was actually seen, the individual wanted record cites that evidence. Where nothing was found, the record says so.

Four copies are still sought (Creekview evaluation, Keys on Freedom, ECPM manual(s), and original ministry CSIL pilot/design records). The November 2008 program-review synthesis is no longer copy-sought: a Microlog fiche is inspectable on Internet Archive (see below). Do not treat a remaining wanted row as an inspected text.

1. Creekview 202 Project Evaluation Report

• Creator as first named for this archive: Patricia Ryan / Ryan & Wiggins
• Date as named: c. 1987
• Independent public evidence now cited: Mattinson and Rompf 1987 (PubMed abstract: formal evaluation completed by the B.C. provincial government and CPA); Haw 1989 (“an independent evaluation of Creekview 202 was published” in 1987). Contemporaneous Creekview setting: Hansard 1984 and 1986.
• Still missing: the evaluation itself; any catalogue record naming Ryan / Ryan & Wiggins; any CMHC bibliography citing this title.
• Leads noted during the Creekview chapter: full Dimensions in Health Service article; library records for Ryan / Ryan & Wiggins; April 1986 ministry correspondence Nielsen mentioned; CPA project files. See the wanted evaluation record.
• Authorship conflict preserved: government-and-CPA (1987 abstract) versus Ryan / Ryan & Wiggins (early project notes) versus unnamed “independent” (Haw).

2. Keys on Freedom

• Full title as named: Keys on Freedom: Resource manual for the development of self-managed community living alternatives for quadriplegic persons
• Creator as named: BC Rehabilitation Society
• Date as named: c. 1990
• Public evidence now cited: none found. Targeted CMHC / publications.gc.ca and bibliographic searches in this pass did not confirm title, author, or year. The Roeher bibliography does not list the title.

3. Enhanced Consumer Participation Model (ECPM) manual(s)

• Creator as named: BC Rehab / Greater Vancouver Home Support Society
• Date as named: c. 1990 and possibly a 1994 training revision
• Public evidence now cited: Roeher / Salisbury bibliography independently cites Enhanced Consumer Participation Model: Skill Development Manual for Consumers of Home Support Services (BC Rehab & Greater Vancouver Home Support Society, 1990; five modules; 52 pages plus appendices). See that finding aid.
• Still missing: the manual itself; any 1994 revision; title-page authorship (including any role for Emma Van der Klift). Van der Klift’s 1994 chapter that we have read is a different work. Current Vancouver Coastal Health CSIL pages do not use the ECPM name. The evidence is not clear enough to say whether a 1994 ECPM manual is a distinct work. Narrative: ECPM chapter.

4. Original Ministry of Health CSIL pilot/design records

• Date as named: approximately 1993–95
• Likely administrative home as named: Continuing Care Division / Community Support Branch
• Public evidence now cited: later accounts disagree — Gauthier 2014 says established 1993; Gauthier 2015 and IFRC Our Team say 1994; Ombudsperson 2012 says pilot 1994, province-wide 1995. HEU Guardian Nov/Dec 1995 calls CSIL a new/pilot program of the Community Support Branch, Continuing Care Division. The exact start date is not completely clear; later official records give 1994 and 1995 different meanings, while a 1995 newspaper still said new/pilot. Narrative: CSIL design and pilot.

UBC, SFU, Legislative Library of British Columbia, and CMHC Housing Knowledge Centre native catalogue interfaces were not successfully queried. They remain useful next checks.

A later 20 September 2026 follow-up opened the Internet Archive Microlog item micro_IA40244204_0972 (Choice in Supports for Independent Living (CSIL) program review synthesis report). The earlier advanced-search note that IA “did not return either title” for the 2008 review was a miss; the fiche is public. Creekview, Keys on Freedom, ECPM, and ministry pilot/design copies were not recovered in that follow-up.
